The Direct Support Professionals provides support to the individuals by attending medical appointments, monitoring their health, safety, nutrition, grocery shopping, community activities, and community connections.

Summary The Employment and Community Direct Support Professional (DSP) provides individualized assistance and support to people with developmental disabilities or acquired brain injury, helping them succeed in employment and become active in their community.
The work of a DSP is varied, creative, flexible, and rewarding. A DSP might support an individual at their place of employment, provide skills coaching, share a meal together, help maintain appropriate personal care, attend community events, and help make decisions about schedules and daily activities. Positions are typically part time with some fulltime opportunities available. Extensive training provided.
Essential Functions
<ol>
<li>Employment and Community Direct Support Professionals (DSPs) provide one-on-one support to help individuals learn and maintain employment skills. These responsibilities include supporting the individual at their place of employment, provide skills development, coaching in communication, problem solving and workplace expectations.</li>
<li>DSPs assist individuals in accessing and engaging in community and social activities. These could include going out to eat, attending concerts or movies, building friendships, volunteering, taking walks, or participating in sports or workouts. DSPs provide transportation to work and community activities, and must have a valid driver's license and reliable transportation.</li>
<li>DSPs support individuals in achieving goals outlined in their service agreements. They accurately track an individual's progress, inform the Program Manager of any changes affecting employment, including wages and hours. They also report concerns affecting an individual's well-being, injury, illness, and health or emotional changes.</li>
<li>DSPs complete required documentation (daily notes, medication logs, incident reports, progress notes, etc.) accurately and on time, and adhere to all regulations, policies and procedures. They follow safety protocols to maintain a safe environment, and report unsafe conditions to the Program Manager. A DSP acts in accordance with legal responsibilities to ensure the individual's safety and welfare and maintains confidentiality.</li>

Mileage reimbursement is available. Medication certification and Safety Care training may be required. Must be able to lift 25+ lbs with assistance as needed. Community Bridges is looking for a full-time Behavioral Home Direct Support Professional in Boscawen, NH!
We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.
The Behavioral Home Direct Support Professionals responsibility is to be a community connector and will be responsible to do community mapping with the individual to promote social inclusion. The Behavioral Home DSP will explore new opportunities for individuals to whom they provide supports and will assist with growth and unity within the community. Promoting abilities not disabilities and to develop a bond between individual and community, family, friends and significant other in their lives. More importantly, to identify those assets that most likely lead to social inclusions for a specific individual whose gifts and interests have been identified.
Qualifications:

Compensation and Benefits:
There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role.

Specific Duties and Responsibilities:

<li>Develop a relationship map to gather information about the individual in order to establish social inclusion opportunities.</li>

<li>Assist individuals in developing a network of people in their lives. Maximize opportunities for the individuals they support with disabilities to get to know others and maximize the opportunities for others to get to know the individual.</li>
<li>Assist the individual in obtaining a sense of self-esteem through active participation in the community.</li>
<li>Provide individualized type and level of support to include, but not be limited to, personal care and/or skill acquisition etc.</li>
<li>Assist individuals in establishing, maintaining, and presenting a positive image. This may involve such activities as assisting in the bathroom, bathing, showering, and dental hygiene, etc.</li>
<li>Assist in developing and individualizing the yearly Person Centered Planning by providing information for assessments and by developing implementation strategies for goals/objectives and methods for monitoring the delivery and quality of supports to the individual.</li>
<li>Complete documentation requirements as specified by the Individual Service Plan to include the scheduled data collection and monthly progress notes to show an increase or decrease in behaviors to continually support individuals as they grow and reach goals.</li>
<li>Assist in the orientation/training of new staff, which may include introduction to an individual's support persons and to an individual's ISP, strategies, dreams and goals, and treatment plan.</li>
<li>Distribute medication in accordance with He-m1201 as applicable.</li>
<li>Report any safety, health or human rights concern immediately to the Supervisor.</li>
<li>Understand and ensure compliance with applicable state regulations.</li>

Work Environment and Physical Demands:

<li>Work environment- The positions is typically indoors, unless the individual wants to go outside. The noise level can be loud for short periods of time, typically in the beginning of the day when individuals are getting ready for their day. There may also be periods of stressful situations for the individuals where staff are expected to provide support.</li>
<li>Physical demands- Direct Support Providers must be physically able to lift up to 50 lbs. and to provide transfers as necessary. Walking and standing required frequently with occasional stair climbing. Must be able to push wheelchairs and operate wheelchair lifts/Hoyer and barrier free lifts. Emergency behavioral intervention must be followed in accordance with behavioral intervention techniques and individual behavioral plans.</li>

Position Summary: To train, coach and support the individuals we serve to promote the highest level of independence, and to

PAY: $19-$21/hr.
 
Essential Responsibilities: 
· Responsible for remaining on shift until adequate coverage can be provided as determined by House Manager or designated responsible person (Senior Person on shift).
· Responsible for consistent implementation of resident programming training (Service Agreement Goals &amp; Objectives: daily living skills, i.e. bathing, dressing, meal preparation; community, access i.e. shopping, leisure activities, concerts, sports, etc.).
· Will provide transportation for residents in vehicles approved by Agency. In emergency situations, staff may be required to transport clients in their own vehicles.
· Responsible for meeting all Residential Standards and Regulations as required by the Division of Mental Health and Developmental Services and/or Moore Center Services, Inc.
· Will participate in the development of resident’s programs, i.e. Service Agreements, Behavior Programs, Strategies and Client Centered Conferences.
· Responsible for aiding in the general upkeep of residence, furnishings and equipment (yard/lawn work, mop, dusting, vacuuming, laundry, housekeeping)
· Upon successful completion of medication training program within one month and subsequent re-certification employee is responsible for supervision of residents’ self-administration and/or the distribution and administration of prescribed medications to residents as designated by the House Manager.
· Responsible for supervision of residents’ medical/evaluative appointments and accompaniment of residents to such appointments.
· Responsible for appropriate record keeping and report writing, i.e. Daily Log content, Resident Goal and Objective documentation, Significant Event Reports, Accident Reports, Behavior Programs, filing along with other reports as needed.
· Will attend the required meetings as requested.
· Responsible for reporting any pertinent client incidents/inconsistencies from routine.
· Staff working in designated residences will complete training behavior management programs prior to working with specific residents. Staff are responsible for consistently implementing and documenting interventions as indicated in programs. See addendum for physical requirements.
· Responsible for always ensuring resident hygiene and appropriate attire.
