Overview
PathWays has been serving individuals with developmental disabilities and their families in Sullivan and Lower Grafton counties of NH for over 50 years. Our Self-Directed Service Coordinators work in partnership with individuals, and those who support them, in designing, implementing, and monitoring supports and services that promote individual choice and decision-making authority. This position requires a dynamic individual with the ability to work collaboratively within an individual’s team, has excellent time management skills, and a passion for supporting individuals to pursue their best lives.
 
Duties
<ul>
<li>Carry a caseload of 25-30 individuals.</li>
<li>Establish and maintain a fluid schedule of quarterly visits and monthly contacts with individuals and families.</li>
<li>Facilitate person-centered annual planning meetings to support individuals in a manner that promotes individual choice and maintains a focus on individuals’ strengths, interests and motivations.</li>
<li>Complete and track all regulatory compliance requirements such as creation of annual service plans, monthly documentation, and quality assurance tracking across several systems.</li>
<li>Work in partnership with individuals and their families to develop individual budgets and to monitor the implementation of those budgets through routine reviews of financial statements.</li>
<li>Provide support to address quality or areas of concern expressed by individuals or their families through cooperation with other team members in a responsive and dynamic manner.</li>
<li>Have a comprehensive understanding of the needs and circumstances of the individuals and families you serve.</li>
<li>Build and maintain an understanding of the Developmental Disability and Children’s In Home Support Medicaid waivers.</li>

Requirements

<li>Bachelor’s degree in human service or related field is highly preferred. </li>
<li>2-3 years of case management or similar experience required. Additional years of relevant experience may be substituted for degree. Previous experience with developmental disabilities is preferred.</li>
<li>Valid driver’s license, proof of personal automobile liability insurance coverage, and access to a reliable vehicle for required regional travel. Mileage reimbursement is available for travel required for the role.</li>
<li>All applicants must complete a comprehensive background check that includes a criminal background check, MVR, BAAS, DCYF, and OIG check.</li>

JOB DESCRIPTION

Summary/Objective Works under general supervision of the IT Field Support Manager. Provides primary support of hardware and software solutions deployed in various retail locations. Expertise to resolve user problems presented from the National Operations Center or any other internal support teams. Provide excellent customer service to market through all channels of support. 

Essential Functions
<ol>
<li>Answers, evaluates, and prioritizes service requests received (but not limited to) companies ticketing system, telephone, email, and in-person. </li>
<li>Assist with identifying problems and providing potential resolutions.</li>
<li>Coordinates with end user on behalf of other departments. Analyze the customers needs and follow approved processes and documentation to meet expected service levels. </li>
<li>Tracks issues using company approved ticketing system. Maintains historical records and related problem documentation.</li>
<li>Work alongside PMO, Asset Management or any other departments to update and maintain hardware and software standards. </li>
<li>Brainstorms with team members and SMEs to resolve more complex problems. Escalate issues to Staff Technician that require further technical assistance in resolution.</li>
<li>Regularly communicates with team and leadership of potential trends, significant problems and individual market requests.</li>
<li>Maintains up to date knowledge of company software, hardware and approved documented procedures.</li>
<li>Schedules preventative maintenance on company owned vehicle. </li>
<li>Escalate and schedules vendor services as necessary.</li>
<li>Installation and troubleshooting of WAN/LAN, Cat 6e cabling.</li>
<li>Manage and track inventory stockroom(s) for designated territory according to team standards.</li>
<li>Responsible for expense records and reports for company provided procurement card.</li>

As a warehouse shipper at our local bakery, you will play a crucial role in ensuring the efficient and timely support of bakery products to various destinations. You will be responsible for unloading, sorting, and staging product for the sales team; often this will be done independently and requires ability to work alone while maintaining a safe and organized work environment. We are currently seeking a Warehouse Associate to work typical hours of 2pm-10pm or until finished. Start and stop times may vary dependent on each warehouses delivery schedule.
