Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness, co-occurring developmental disabilities and substance use disorders. Assertive Community Treatment (ACT) is an evidenced based practice model that provides an intensive and highly integrated approach to community mental health services. ACT support helps people with severe and persistent mental illness become independent and live in the community as they work toward their recovery goals. ACT support also helps with reducing homelessness and unnecessary hospital stays. 
 
The Adult ACT Medication Training and Support (MTS) Specialist provides outpatient community-based counseling support to educate; problem solve and develop strategies to incorporate prescribed medication into daily routine and lifestyle. Additionally, they will assist clients with improving mental and physical health symptoms and behaviors that otherwise interfere with daily functioning and wellness goals. This is a per diem position - hours vary based on need.
 
Major Functional Areas and Description of Duties

<li>Provide Medication Training and Support (MTS) services to educate, problem solve and develop strategies to incorporate prescribed medication into daily routine.</li>
<li>Deliver counseling services in the home or other community-based setting as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan.</li>
<li>Develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement.</li>
<li>Provide Family Training and Counseling (FTC) services, as needed, to educate natural supports and promote how to best encourage the client’s resiliency and recovery toward treatment goals.</li>
<li>Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.</li>
<li>Work collaboratively with other ACT Team members, natural supports, and external providers to ensure wrap-around services are in alignment with the ACT model.</li>
<li>Adhere to and uphold the General Organization Index and Fidelity Scales specific to IMR and ACT to support the authenticity of program implementation.</li>
<li>Serve on appropriate committees, both internal and external, as determined by supervisor.</li>
<li>Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients.</li>
<li>Execute all required services and timely documentation in accordance with agency policies and federal/state standards.</li>
<li>Maintain professional memberships, licenses, accreditation, certifications, etc.</li>

She does exhibit intense behavioral outbursts. Becca has a close relationship with her family and likes to socialize with friends whom she has maintained relationships with from Special Olympics'. Becca also attends equine therapy where she helps care for the horses and enjoys going out to eat and shopping.
She is mobile but requires a wheelchair for long distances as she tires easily and is at high risk for drop seizures. She also utilizes a gait belt/harness for walking and transfers, as she has an extensive seizure history. Behavioral challenges along with ongoing seizure activity continue to be an issue for Becca's day-to-day living and functioning. She requires personal care support from her home care provider and staff but has been working on her independence with her continence and hygiene. Becca uses some verbal communication and picture exchange communication (PECS) and visual board communication but requires support in communicating.

Responsibilities:
<ul>
<li>Assist clients with ADLs such as toileting, bathing, hygiene care, dressing, transferring, mobility, meal preparation, transportation, and companionship!</li>
<li>Possess a current and valid driver’s license, a reliable vehicle, and current and valid car insurance</li>
<li>Ability to lift at least 25lbs</li>
<li>Willing and able to commit to a weekly schedule of at least 20 hours</li>
<li>Participate in criminal and motor vehicle background checks</li>
<li>Provide care according to Home Instead company standards</li>
