Becca is a passionate, playful, and genuine 31- year-old woman. She loves her animals, and they sometimes help regulate her anxiety at home and in the community. When at a behavioral baseline, Becca enjoys social interactions with others, especially those close to her, and with whom she has formed relationships within her social circles. She does exhibit intense behavioral outbursts. Becca has a close relationship with her family and likes to socialize with friends whom she has maintained relationships with from Special Olympics'. Becca also attends equine therapy where she helps care for the horses and enjoys going out to eat and shopping.
She is mobile but requires a wheelchair for long distances as she tires easily and is at high risk for drop seizures. She also utilizes a gait belt/harness for walking and transfers, as she has an extensive seizure history. Behavioral challenges along with ongoing seizure activity continue to be an issue for Becca's day-to-day living and functioning. She requires personal care support from her home care provider and staff but has been working on her independence with her continence and hygiene. Becca uses some verbal communication and picture exchange communication (PECS) and visual board communication but requires support in communicating.

Center for Life Management (CLM) provides the area's most comprehensive array of mental health services. Our Collaborative Care Program is an innovative approach to healthcare that encompasses the integration of mental, physical and behavioral health needs. Working together as a multi-disciplinary team, internal (CLM) and external (area agencies) work as one entity to support individuals with co-occurring mental health needs and intellectual disabilities and/or traumatic brain injury. The Collaborative Care Community Support Counselor provides outpatient community-based support to assist client’s with improving symptoms and behaviors that otherwise interfere with daily functioning and personal wellness goals. This is a full-time, 35 hour per week position. 
 
Major Functional Areas and Description of Duties

<li>Provide psychoeducation using the evidence-based practice of Illness Management and Recovery (IMR) to support goal and coping skill development. </li>

The Center for Life Management has partnered with New England College to offer a Master’s Degree! This is an exciting opportunity for CLM staff to earn a Master of Science in Clinical Mental Health Counseling (MS) degree in 3 years.
Who’s Eligible and What’s Required:
<ul>
<li>Must be a regular, full-time CLM employee in good standing for 9 months or more.</li>
<li>Must hold a Bachelor’s Degree.</li>
<li>Must agree to a five (5) year commitment of full-time employment in good standing with the Center for Life Management (beginning from start date/year of Professional Development Program participation).</li>
<li>Participants must maintain a B average to continue participation in the program.</li>
</ul>

Requirements:
Qualifications and Skills
<ul>
<li>Bachelor’s degree preferred however an Associate’s degree along with a minimum of two years of direct service in a related field may qualify.</li>
<li>Experience working with individuals with intellectual disabilities and/or traumatic brain injury is preferred.</li>
<li>Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations.</li>
<li>Proficient computer skills, specifically with all Microsoft office applications.</li>
<li>Ability to document effectively and efficiently in course of clinical work.</li>
<li>Ability to navigate an electronic health record. </li>
<li>Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision.</li>
<li>Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services.</li>
<li>Demonstrate effective communication skills, organizational skills, and ability to multi-task.</li>
