During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:

<li>Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Administer and order point of care and lab-based testing as needed</li>
<li>Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary</li>
<li>Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities</li>

<li>Executive Chef - In the absence of the Account Manager, the Executive Chef assumes the responsibility of managing and supervising the dining services staff. Responsibilities

<li>Assist with food preparation, including chopping vegetables and preparing salads.</li>
<li>Serve meals to residents according to dietary needs and portion sizes.</li>
<li>Clean and sanitize kitchen equipment, utensils, and dining areas.</li>
<li>Follow food safety and sanitation guidelines.</li>
<li>Interact positively with residents, staff, and guests, providing excellent customer service.</li>

Qualifications
<ul>
<li>High school diploma or equivalent preferred.</li>
<li>Previous experience in food service or kitchen environment is preferred but not required.</li>
<li>Ability to follow instructions and work as part of a team.</li>
<li>Maintain a positive attitude and effectively communicate with building occupants, administration, and HCSG leadership to perform daily tasks.</li>
<li>Compliance with COVID-19 vaccination policies</li>
<li>Must be able to lift/carry a maximum of 30 pounds, push/pull a maximum of 50 pounds, stand, sit, bend and walk for extended periods. </li>
<li>Must be able to perform routine, repetitive tasks continuously.</li>
<li>Must be able to work around food and cleaning products.</li>
<li>Current ServSafe or Food Handler certification is required based on State / County law. </li>
<li>Residency within the service area required</li>
