Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness, co-occurring developmental disabilities and substance use disorders. The Adult Therapist provides diagnostic assessment, treatment planning, therapy, and crisis management. This is a full time, 40 hour per week position. This is a hybrid position, living locally within our facility is required to be able to serve our community.
 
Major Functional Areas and Description of Duties

<li>Provide individual diagnostic assessment to determine client’s strengths, needs, resources, and objectives;</li>

The Integrated Care team provides a full wrap-around care model that is necessary to support individuals with addressing both their significant mental and chronic physical health conditions. The Integrated Care Whole Health Clinician provides initial and ongoing diagnostic assessment, treatment planning, therapy, and crisis management to help client’s improve medical and mental health symptoms and behaviors that otherwise interfere with daily functioning and personal wellness goals. This is a full time, 35 or 40 hour per week position.
Major Functional Areas and Description of Duties of the Integrated Care Behavioral and Physical Clinician.

Responsibilities

<li>Perform daily housekeeping tasks, including cleaning resident rooms, bathrooms, common areas, and offices.</li>
<li>Ensure proper cleaning and sanitation of all areas following established procedures and safety guidelines.</li>
<li>Maintain records of cleaning activities and report any maintenance or safety issues to the supervisor.</li>
<li>Follow infection control and universal precautions policies to ensure a sanitary environment.</li>
<li>Interact positively with residents, staff, and guests, providing excellent customer service.</li>
<li>All other duties as assigned.</li>

Qualifications
&nbsp;

<li>High school diploma or equivalent.</li>
<li>Previous housekeeping experience is preferred but not required.</li>
<li>Maintain a positive attitude and effectively communicate with building occupants, administration, and HCSG leadership to perform daily tasks.</li>

CLM provides the area's most comprehensive array of mental health services for children and their families. Our multi-disciplined treatment team works with individuals ages 3 – 21 who experience mental illness and/or serious emotional difficulties. The Children’s Clinician provides outpatient clinical services including diagnostic assessment, treatment planning, crisis management, individual therapy, family therapy, and group therapy. This is a full time, 35 hour per week position and is considered exempt in accordance with the Fair Labor Standards Act (FLSA).
 
Major Functional Areas and Description of Duties
<ul>
<li>Provide individual diagnostic assessment to determine client’s strengths, needs, resources, and objectives.</li>
<li>Formulate treatment plans specifying presenting problems, objectives, interventions, and desired outcomes.</li>
<li>Complete assessments for eligibility determination for State funded services.</li>
<li>Provide individual, family, and group therapy in the office and/or remote via telehealth.</li>
<li>Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community. </li>
<li>Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around family systems approach is provided to the client.</li>
<li>Provide therapy service each week (eligible for incentive pay upon exceeding productivity expectation).</li>
<li>Serve on appropriate committees, both internal and external, as determined by supervisor.</li>
<li>Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients.</li>
<li>Execute all required services and timely documentation in accordance with agency policies and federal/state standards.</li>
<li>Maintain professional memberships, licenses, accreditation, certifications, etc.</li>
<li>Represent CLM in a professional and appropriate manner in all settings.</li>
<li>Perform all other duties as assigned by supervisor.</li>

Requirements:
<ul>
<li>Master’s degree in clinical mental health counseling, social work or psychology required.</li>
<li>LISCW, LCMHC or LMFT preferred.</li>
<li>Experience working with co-occurring disorders preferred.</li>
<li>Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations.</li>
<li>Ability to actively engage with children and conduct interactive play within a therapeutic setting.</li>
<li>Proficient computer skills, specifically with all Microsoft office applications.</li>
<li>Ability to document effectively and efficiently in course of clinical work.</li>
<li>Ability to navigate an electronic health record. </li>
<li>Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision. </li>
<li>Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services.</li>

Physical Demands and Working Conditions
A majority of the work will be in the office and/or remote via telehealth. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable internet access is required.
