Community Bridges is seeking a full-time Residential Program Manager – Medical to join our team in Northfield.

Founded in 1982, Community Bridges is a nonprofit organization dedicated to advancing the integration, growth, and interdependence of people with developmental disabilities within their communities. We proudly support more than 1,100 individuals and their families throughout the Greater Capital Area by identifying barriers to access and developing creative, person-centered solutions.

About the Medical Residence

In our Medical residences, we support individuals with developmental disabilities and/or complex medical needs requiring consistent daily care, as well as occasional behavioral supports. Individuals maintain structured daily routines, participate in meaningful community activities, and enjoy learning and exploring new experiences.

The Residential Program Manager – Medical oversees all aspects of a certified residential program, ensuring high-quality, person-centered services while maintaining compliance with DMHDS certification standards and agency policies. This role includes operational oversight, staff supervision, and clinical/medical coordination.

Qualifications

Education

  • Bachelor's Degree preferred

  • Associate's Degree with three (3) additional years of relevant experience considered

Experience

  • Minimum of one (1) year working with individuals with disabilities

  • Minimum of two (2) years of supervisory and administrative experience

Other Requirements

  • Reliable transportation

  • Valid driver's license

  • Proof of automobile insurance

Compensation & Benefits

???? Sign-On Bonus

  • $1,000 total

    • $500 paid at hire

    • $500 paid after 6 months of employment

  • Sign-on bonuses are for new hires only. Rehires and transfers are not eligible.

Key Duties & Responsibilities

  • Provide individualized support, including direct care, personal care, and skill acquisition

  • Support individuals in accessing community resources and building social connections

  • Promote independence, dignity, choice-making, and positive self-image

  • Assist with personal hygiene and daily living activities as needed

  • Recruit, hire, train, supervise, and evaluate residential staff

  • Oversee residential operations, staffing coverage, and documentation

  • Develop, implement, and monitor program budgets and individual service plans

  • Ensure compliance with state regulations and agency policies

  • Coordinate with families, medical providers, and interdisciplinary teams

  • Attend service plan meetings, staff meetings, and agency trainings

  • Distribute medications in accordance with He-M 1201

  • Participate in rotating on-call coverage

  • Provide direct service coverage as needed

  • Perform additional duties as assigned

Required Skills

  • Strong verbal and written communication skills

  • Ability to advocate for individuals and support self-advocacy

  • Ability to collaborate with families, team members, and community partners

  • Team-oriented approach with strong leadership skills

  • Ability to complete and maintain required certifications (Medication Administration, MOAB, First Aid/CPR)

  • Basic computer competency

  • Flexibility to work occasional evenings and weekends

Physical Demands

  • Ability to lift up to 25 pounds

  • Frequent standing, walking, and stair use

  • Ability to push wheelchairs and operate lifts (Hoyer/barrier-free)

  • Ability to participate in emergency behavioral interventions as trained

Travel Requirements

  • Frequent travel during scheduled work hours using personal or agency vehicles

  • Must maintain a valid New Hampshire driver's license, safe driving record, and adequate insurance
