Our Integrated Care Program is an innovative approach to whole healthcare that encompasses the integration of treatment modalities for mental and physical health needs. Working together with a multi-disciplinary team, the Wellness Coach, a cross-trained mental health counselor and certified personal trainer, provides outpatient community-based support to assist client’s with improving symptoms, behaviors, and co-morbid disabilities. This is a 40 hours per week position.
 
Major Functional Areas and Description of Duties
<ul>
<li>Provide Functional Support Services (FSS) in the home and community-based settings as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan.</li>
<li>Engage client in the process of working toward and reaching their personal health, wellness and recovery goals by using motivational strategies and interventions.</li>
<li>Engage client in applicable assessments to monitor symptoms and social determinants of health.</li>
