Key Responsibilities

1. The Case Manager will provide TCM to Eligible clients on their caseload as needed per their assessment and care plan on a monthly basis. If service is not provided, outreach attempts are documented.

1. The Case Manager will engage in caring and assertive outreach to Eligible Meds Only Outreach clients who are not engaging in recommended services, in attempts to aid them in breaking down barriers for success in treatment. TCM will be provided and if service is not provided, outreach attempts are documented.

1. The Case Manager will be available to Eligible Meds Only Relapse Prevention clients who are stable, engaged in psychiatry and whom may have TCM needs or needs that may arise in the future.

1. The Case Manager is a part of the client’s treatment team and is responsible for relaying information through the appropriate channels. This may include attendance at team meetings or consults with other providers as needed.

1. The Case Manager is required to attend supervision, team meetings and in-services on a regular basis, as designated by the Supervisor.

1. The Case Manager must be aware of and adhere to all SMHC Policies and Procedures.

1. The Case Manager will be aware of and adhere to state and federal mandates regarding service provisions.

1. The Case Manager is required to keep regular documentation of all activities. This may include TCM notes, Nonbillable Contact Logs, TCM Assessments and Care Plans, Quarterly Treatment Plan Reviews, ANSA Annual and Eligibility Assessments, and referrals for additional SMHC services.
