Job DescriptionSalary:
Recovery Support
- Basic screening of persons with substance use disorders and cooccurring mental health disorders to determine whether referral for further assessment and formal diagnosis is needed and the making of appropriate referrals.
- Provides practical support, mentoring, and education about addiction, community peer support, role of medication and cooccurring disorders in addiction.
- Monitoring of clients' health and safety including basic recognition of signs and symptoms of addiction, intoxication, and withdrawal, and the implementation of structured interventions to ensure the immediate safety of clients demonstrating such symptoms
- Advocates for individuals and assists them in achieving their treatment goals
- Identifies and builds relationships with community wide services
- Maintains documentation, complies with HIPAA privacy laws, and ensures compliance with federal, state and local regulations
- Maintains working relationships with staff and other agencies or institutions to provide awareness of recovery and the availability of CRSW
- Provides outreach in the local area to promote the recovery coach services and supports business development initiatives as appropriate
- Connects individuals to personal and community support necessary to live independently.
- Assists individuals with obtaining recovery-oriented materials, attends recovery support meetings, and provides referrals, as necessary
- Runs recovery-based psychoeducation groups for clients in IOP and OP
- Participates as part of an interdisciplinary team
- Provides case management services (applying for housing, funding, food stamps, finding employment, setting up doctor's appts, setting up transportation, supporting legal needs, referral to higher LOC, etc.).
- Supports clients with activities of daily living
- Provides leadership to sober living houses and assists with screening sober living applicants, as well as supports residents in sober living with obtaining recovery capital
- Provides admission support to residents admitting to sober living
Sober Living Coordination:
- Serve as the primary liaison between the clinical, medical, housing teams, and clients' families, ensuring all needs are met during the clients stay in
sober living.
- Oversee the coordination of sober living housing, ensuring residents are following house rules and addressing any issues that arise.
- Coordinate transportation and travel services for clients as needed, including for admissions, appointments, or family visits.
- Assist in managing the day-to-day operations of the sober living facility, including scheduling, maintenance needs, and resident support.
- Provide guidance and support to residents to help them adjust to the sober living environment and transition to independent living.
- Organize and maintain records related to sober living residents, including assessments, progress, and any incidents.
Client and Family Communication:
- Communicate effectively with clients, families, and external agencies to ensure a smooth admission process and successful transitions in sober living.
- Provide families with regular updates on their loved ones progress and help manage expectations throughout the treatment and sober living process.
- Data and Documentation:
- Compile and report statistics relevant to admissions and sober living operations.
- Maintain accurate and up-to-date records in compliance with all applicable regulations and accrediting agency standards.
- Professional Representation:
- Represent the program professionally to referral sources, potential clients, and other outside agencies.
- Collaborate with clinical and administrative teams to ensure the highest level of care and support for all clients.
Other Duties:
- Available to be on-call during evenings and weekends via telephone, if required.
