Job Description:
As a Physical Therapist (PT), you will play a critical part in improving residents' mobility, strength, and overall well-being. You will assess, plan, and implement therapy programs tailored to each individual's needs while maintaining the highest standards of care.

Key Responsibilities:

  • Perform initial and ongoing assessments of residents to determine their physical therapy needs.
  • Develop and implement individualized treatment plans focused on achieving measurable outcomes.
  • Use a variety of techniques to improve residents' physical function and prevent further injury.
  • Collaborate with all departments, including occupational therapists, nurses, and physicians, to ensure cohesive care.
  • Educate residents and families on best practices for mobility and rehabilitation.
  • Maintain accurate and timely documentation.
  • Provide progress updates as needed.


Qualifications:

  • Must be a Physical Therapist (PT) with a valid state license
  • Excellent interpersonal and communication skills.
  • Compassionate and patient-centered approach to therapy.
  • Ability to work effectively in a team environment.
  • Strong organizational skills and attention to detail.
