Licensed Practical Nurse (LPN)

Facility: Harris Hill Center
Shift: 1st Shift (6:45 AM – 3:15 PM)

Position Summary:

Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) delivers efficient, high-quality nursing care while achieving positive clinical outcomes and patient/family satisfaction. The LPN operates within the scope of practice defined by the State Nurse Practice Act and contributes to nursing assessments, care planning, and supervision of unlicensed staff to ensure optimal resident care.

Responsibilities and Accountabilities:

1. Patient Evaluation:

    Collects, reports, and documents objective and subjective patient data.

    Observes changes in patient conditions and promptly reports to the RN.

2. Care Planning:

    Contributes to developing individualized patient goals.

    \t

    Assists in formulating interventions to meet care objectives.

    Implements the plan of care and evaluates its effectiveness.

    \t

    Participates in revising the plan of care as needed to prevent rehospitalizations.

3. Direct Patient Care:

    Administers medications and performs treatments per physician orders.

    \t

  • \t

  • \t

    Communicates pertinent patient information to the RN and/or physician.

    \t

  • \t

  • \t

    Accurately and thoroughly documents all care activities.

    \t

  • \t

  • \t

    Seeks guidance from the RN as necessary.

    \t

  • \t

  • \t

    Participates in interdisciplinary care meetings to support effective discharge planning.

    \t

  • \t

  • \t

    Educates patients and families regarding discharge instructions and ongoing care.

    \t

4. Supervision of Unlicensed Staff:

    Assigns and monitors tasks delegated to unlicensed personnel per policy and training.

    \t

  • \t

  • \t

    Ensures that care is delivered in compliance with standards and regulations.

    \t

  • \t

  • \t

    Provides guidance and feedback to support staff performance.

    \t

  • \t

    Facilitates effective communication between shifts.

    \t

5. Documentation and Systems Use:

    Utilizes Point Click Care (PCC) for accurate charting and record management.

    \t

  • \t

  • \t

    Performs drug counts at shift start, end, and as required.

    \t

6. Collaboration and Communication:

    \t
  • \t

    Assists physicians during rounds and examinations as needed.

    \t

  • \t

  • \t

    Collaborates with other departments to coordinate patient-centered care.

    \t

  • \t

  • \t

    Participates in effective shift-to-shift handoffs.

    \t

7. Professional Development and Compliance:

    Attends all mandatory in-service and training programs.

    \t

  • \t

  • \t

    Assists in orienting and mentoring new nursing staff.

    \t

  • \t

    Promotes a culture of safety and quality improvement.

    \t

  • \t

  • \t

    Maintains patient confidentiality in accordance with HIPAA standards.

    Complies with legal and ethical requirements, including the Compliance and Ethics Program and Federal False Claims Act.

8. Additional Duties:

    Remains available to work beyond scheduled shifts as required to meet patient care or staffing needs.

    Performs other related duties as assigned by the RN or Nurse Manager.

Educational / Vocational Requirements:

    \t

    Graduate of an approved school of practical or vocational nursing.

    \t

  • \t

  • \t

    Current and active LPN license issued by the State Board of Nursing.

    \t

  • \t

  • \t

    Prior experience in geriatric or rehabilitative nursing preferred.

    \t

  • \t

  • \t

    Must be able to read, write, and communicate effectively in English.

    \t

  • \t

  • \t

    Must provide verification of a current Tuberculin Skin Test (TST) per state law.

    \t

  • \t

  • \t

    Must maintain a valid BLS/CPR certification.

    \t
