Exciting Full-Time School-Based Case Manager Position in Laconia, New Hampshire!

We are seeking a passionate Case Manager to join our team and support students in a school setting for the upcoming school year. If you have experience in case management and are ready to make an impact in the lives of students, wed love to hear from you!

Responsibilities:

• Coordinate services and develop individualized education plans (IEPs) for students with special needs.

• Collaborate with teachers, parents, and specialists to ensure comprehensive support for students.

• Monitor student progress and adjust services to meet evolving needs.

• Provide resources and guidance to families to enhance educational outcomes.

Qualifications:

• Active State Licensure or Certification in New Hampshire.

• Previous school-based experience in case management or a related field.

• Bachelors or Masters degree in Social Work, Psychology, or a related field.

Position Details:

• Classroom: K-12

• Hours per Week: Full-Time (35-40 hours)

• Weekly Pay: Competitive pay based on experience

Contract Duration:

• Current school year with the option to extend for summer school and the 2024-2025 school year.
