Licensed Practical Nurse/Scheduling Center
Full Time, Monday-Friday
Grace Cottage Family Health and Hospital currently has an opening for a full-time LPN/Scheduling Center position for Grace Cottage Family Health Clinic.
Responsibilities include: handling incoming calls from patients, family members, and internal/external customers, and documenting these conversations in patients electronic medical records, and scheduling appointments. Also intakes patient information needed for appropriate care or services, using nursing skills and critical thinking skills within scope of licensure. Utilizes a team-based approach by collaborating with providers, nursing staff, and other ancillary staff members within the organization. Collaboration consists of communicating via EHR, directly, or via phone. Maintains confidentiality, HIPAA and PHI compliance.
Must be a graduate of an accredited school of practical nursing, possess a current VT LPN license in good standing, and have excellent customer service skills and proficient computer skills. Patient scheduling experience and 5+ years experience working in primary care setting or in a provider practice preferred.

About this company
When you are hospitalized at Grace Cottage for acute care for illnesses such as pneumonia or congestive heart failure, you will have the benefit of a full-time hospitalist.
Hospitalists manage and coordinate your care while you are in the hospital. These physicians work in the hospital, which allows them to follow up on tests and adjust your treatment throughout your stay as needed. Your Hospitalist will meet with you and your family to provide updates on your care. Your Hospitalist will discuss with you any lab results, changes in medication, discharge needs, and follow up.
A Grace Cottage Hospitalist is on duty during the day, every day of the week. The Hospitalists serve one-week rotations, and so you may meet one or more of our physicians during your hospital stay.Your Hospitalist does not replace your primary care physician (PCP).
Your Hospitalist will communicate regularly with your PCP to ensure that your PCP is informed of your treatment plan and progress. Once you are discharged from the hospital, you will return to your PCP for your medical care.
